IRA Distribution Request

Requirements

To receive your IRA distribution, please select one of the following options below.

Option 1: Rollovers

• Completed IRA Rollover Request Form

Option 2: Distributions

• Completed IRA Lump Sum Distribution Request Form

Required for All Distributions

• Completed W-9 Form
• Completed IRA Simplifier® - Individual Retirement Account Application
• Photocopy of one (1) Identification Form (Driver's License, Military ID, etc.)

A distribution may not be made until all requirements are met and forms are received, complete and accurate.

Information can be faxed to 703-883-3814. Please note that only a clear photocopy of an identification will be readable through fax.
